Claims

To report an incident or claim, complete the appropriate form and forward it to our office. When possible, the loss notice should be faxed to our office at (573) 751-8276. Property claims should be reported using ACORD forms and must be submitted by fax or mail.


You will notice on the right hand side of the form, two (2) boxes marked INCIDENT ONLY and NOTICE OF CLAIM. The same form is used for reporting incidents and claims by marking the correct box. If you want our office to take action on a matter, it must be marked NOTICE OF CLAIM. INCIDENT ONLY reports are not acted upon by our office but instead are kept as a record of a potential claim.

A report should always be completed for the following types of incidents:

  • Any third party bodily injury or death
  • Any property damage of a third party
  • Any slip, trip or fall incidents - whether or not bodily injury is apparent
  • Any automobile accident involving a covered vehicle - regardless of fault
  • Any complaint about your policies and procedures
  • Any allegations of violations of civil rights
  • Any other accident you feel may result in a claim

These types of incidents should be reported to our office as claims when the party involved has indicated that they expect payment for their damages.

IT IS ESSENTIAL THAT ALL CLAIMS BE REPORTED TO OUR OFFICE PROMPTLY. DO NOT WAIT TO SUBMIT THE CLAIM UNTIL YOU HAVE THE POLICE REPORT AND/OR ESTIMATES.


These can be sent to us under separate cover.

It is equally important that incidents of the above nature be reported promptly. Not all incidents result in claims but all claims do start as incidents. By recording and reporting incidents we can work together to identify and alleviate problems before they turn into claims. A reduction in claims will save you money so it is certainly well worth the time to complete the appropriate claim form.

Please do not hesitate to report the type of incidents we have indicated. The number of incidents you report will not affect your annual contribution.

If you are uncertain whether or not to report something to us, report it. We would prefer receiving "unnecessary" reports as opposed to not receiving the ones that are necessary.
